All written receipts and work orders must be in english.
All documents must be originals and not copies.
Titles from Kentucky , Louisiana , North Carolina, Ohio, Oklahoma, Wisconsin need to be notarized.
DEALER
The first time you attend our facility you must bring the documents listed below to add your company to our system. After that, you only need to bring the documents for the vehicle.
Your dealership must be Florida based to conduct a rebuilt inspection.
First Time Attending:
Copy or photo of dealership owner's license
Dealer VI number
FEID/FEIN number
Sales Tax number
Vehicle Information:
Title of vehicle
Bill of sale
5 Photos of the vehicle before any repairs - 4 photos from each side of the vehicle and 1 of the inside showing any deployed airbags
Original receipts for any major components replaced on the vehicle
Body shop work-order if necessary
PRIVATE
You must be a resident in the state of Florida and have a valid, unexpired drivers license or Florida ID to conduct the inspection.

FORMS
Here are important forms you may need to conduct your inspection.
STATEMENT OF BUILDER
This is a mandatory form used to describe the work that was done on the vehicle, including the parts repaired, replaced and where they were purchased.
This form is used as proof of purchase by an individual if you've purchased a vehicle from another private individual. This form cannot be used by dealers.
This document is used to have another authorized individual conduct the inspection in your place. With the details provided and the original signature of owner, authorization is given to the individual to conduct the inspection in your place.
